1. Request for analysis:
This is to request analysis for Fecal Coliform via
Standard Method 9222d for up to 30 samples per day
to be delivered to your lab by PRIDE Clean Streams
Volunteer runners on April 14-15, 2004 

2. Sample Volumes:
The actual number of samples delivered to your lab
may vary but should not exceed that number.

Karen Deaton of PRIDE Inc is the sampling event 
coordinator.  She will have information concerning
what samples will be delivered to your labs.

3. Sample Containers:
Standard 150ml sample containers are being distributed
to school science programs throughout the PRIDE Region.

4. Sample Tracking:
Each sample has a Sample ID code (P001-P999) and a
chain of custody record that matches that Sample ID
Code.

5. Sample Delivery:
Sample runners are to deliver samples within five 
hours of collection.  Samples will be preserved on
wet ice at or near 4 degrees centigrade.

6. Collection of Documents:
We request that you collect the original Chain of 
Custody Records from the sample runners. Also, 
We request you collect any other reports, photographs
or materials turned in by the sample runners.

Please transmit all chain of custody records, reports and other materials received
from the sample runners to:

7. Results Reporting:
We request you report the results of analysis via
a standard Microsoft Excel template found at this address:

8. Price for analysis and Payment
Once analysis is complete, we request you transmit
your invoice, a hard copy report of results to:

PRIDE Inc. will pay up to $15.00/sample for this analysis.
If there are other charges or costs involved, please
let us know prior to the 10th of April,2004.

9. Results Reporting Timetable:
Our hope is to get the electronic results back to the participating teachers and 
students as soon as possible after the sampling event. so the faster
we get the numbers, the more it will mean to the students! (Hopefully within two weeks)
